Clinic Midterm Competency Evaluation

PROFESSIONAL BEHAVIOR
SATISFACTORY - Good attendance, always prepared for treatments. Has developed good rapport with client & caregiver. Great enthusiasm & positive attitude. Appears confident throughout sessions. Works well with peers/colleagues to provide optimal level of care.

EVALUATION OF CLIENT
SATISFACTORY - Completed an accurate evaluation & utilized appropriate assessments. Utilized instructor during difficult assessment areas, able to accurately determine deficit areas & prioritize appropriately. Continue to practice evaluation skills by evaluating clinical & performance areas & documenting on progress notes (ie. AROM of UE's). Commitment shows in every treatment completed!

COMMUNICATION SKILLS
SATISFACTORY - Able to effectively communicate with client, always addresses him & attempts to obtain information from him directly prior to asking caregiver & utilizes effective strategies to communicate with him & elicit responses. Documentation improving. Remember to focus on clear, concise, measurable status/goals in order to show progress in all areas of function.

TREATMENT APPLICATION
SATISFACTORY - Incorporates both pre-functional & functional activities with each treatment. Choosing meaningful, functional & appropriate activities for client. Very creative & utilizes a variety of communication interventions with client/s. During group considered different methods to provide instructions & had them available to meet clients' needs.

ADMINISTRATIVE SKILLS
SATISFACTORY - Appropriately seeks guidance when needed. Researches treatment ideas. Always prepared, with a plan. Uses time effectively. Cleans workspace.

PROFESSIONAL GROWTH & ATTITUDE DEVELOPMENT
SATISFACTORY - Seeks assistance, asks questions, implements feedback well. Incorporates IA's ideas & feedback into sessions. Positive attitude & enthusiasm which motivates client & keeps him engaged.

EVALUATION SUMMARY
Overall developing sound clinical & documentation skills needed for upcoming internship. Treatment interventions have been consistent & conducive to client's progress. Focus on evaluation skills & documenting in a clear, concise & measurable way. Remember to "switch" clients once or twice in order to have the opportunity to work with clients with different deficits & needs.
